The Chiro-Sleeper is the only fully customizable mattress available that is made to order based on your complete specifications.  While we do offer the Chiro-Sleeper 8000 model as described on our website, often times a variation of this model or a complete different configuration may be called for in certain situations.  We are able to make these determinations with the help of your doctor and the very specific questionnaire we ask that you fill out.  Below you will find a downloadable copy of this questionnaire.  Please fill it out and have your doctor sign it.  It can than be faxed or emailed to us.
